Eerstelijnszone Brugge

The primary care network that connects GPs, nurses and care providers across Bruges — and helps residents find the right care.

  • Primary care network
  • For residents
  • For professionals

What we coordinate

Five things residents and care professionals in the Bruges region ask us for by name.

Most asked for

Care coordination for complex situations

When several providers are involved around one person at home, we bring them round one table and write down who does what.

  • A single contact point for the household
  • A multidisciplinary consultation
  • A written care agreement
  • Review when the situation changes

Chronic illness and home care support

Practical support for people living at home with a long-term condition, and for the family members doing the day-to-day work.

  • Mapping the care already in place
  • Links to home nursing and home help
  • Informal carer guidance
  • Handover when care moves between providers

Information and guidance for residents

Plain answers to the question everybody starts with: who do I go to with this, and where are they?

  • Which service fits your question
  • Where to go outside practice hours
  • Help understanding a referral
  • Pointers in clear language

Prevention and health promotion

Local campaigns and neighbourhood initiatives run together with practices, pharmacies, schools and community organisations.

  • Screening and vaccination drives
  • Neighbourhood health sessions
  • Materials for practices and schools
  • Reaching people care usually misses

Network support for professionals

For GPs, nurses, pharmacists, therapists and social workers in the zone: shared agreements, referral routes and a mobile app for finding primary care services nearby.

  • Up-to-date provider directory
  • Agreed referral routes
  • Working groups across disciplines
  • Mobile access to the service map

Eerstelijnszone Brugge does not replace your GP, your nurse or emergency care — we make sure the people already around you are working from the same picture.

How an enquiry goes

  1. You describe the situation

A resident, a family member or a care professional sends us the question in a few lines. No file number needed, no forms to print.

  2. We map what is already there

A coordinator checks which GP, nurse, pharmacist or service is already involved, so nothing gets duplicated and nothing falls between two people.

  3. The right people meet

Where a situation needs more than one discipline, we organise a consultation with the providers involved and, where possible, the person themselves.

  4. You get it in writing

Agreements are written down: who does what, who to call first, and when it gets looked at again. The network stays reachable if the situation changes.

Ask the Bruges primary care network

Residents and care professionals use the same form. Tell us what the question is about and a coordinator picks it up.

  • A coordinator from the network reads every message personally.
  • You get an answer by email saying what we can do and who takes it on.
  • If your question needs urgent medical care, we will point you straight to the right service instead.
